Our Extra Miles, Hospital to Home Support service provides practical support for vulnerable people leaving hospital

We assist with hospital discharge and ensure welfare in the home for the next 7 days

We operate from Leighton Hospital and Macclesfield General Hospital, Monday to Friday

The service is run by St. Paul’s Centre, in partnership with Cheshire East Council
Who can use the service?
Anyone registered with a GP in Cheshire East who has limited support from family or friends

What support is offered?

Transport home followed by 7 days of well-being checks

Home environment safety check – we will also ensure you are warm and comfortable!

Help with essential shopping

Transport to medical appointments

Support with meal arrangements, area dependent

Advice with emergency pendants and key safes

Help with applications for Attendance Allowance and other benefits

Help with access to community activities

We offer a personal and confidential service, with practical and emotional support for those experiencing isolation and loneliness

Our staff can make daily visits or phone calls for 7 days following discharge
